Maui Prep Athletics Physical Day
For all Upper School students for eligibility to compete in HHSAA athletics
Physicals will be held Friday, August 1st starting at 4:30 pm and continue in 10 minute slots until 6:30pm. If all current slots fill up we will add slots on an as needed basis.The cost of physicals is $75, and unfortunately is not covered by insurance.
There are two appointments available for each time slot. Though we will be completing physicals at Maui Prep, the physical form will still need to be uploaded online to SWOL. As a reminder, the athletic department is not accepting any paper forms; however, I will have copies of the physical form available for your convenience. Please let me know if you have any questions in the meantime.
For those choosing to complete physicals through their own physician: Please be sure the physical is completed on the Hawaii Department of Education Athletic Physical Form (available for download on your SWOL account & attached below) by a licensed healthcare provider (medical doctor, osteopathic physician, advanced practice registered nurse, and/or physician assistant). Athletic Physicals are only valid for 1 year from the date of the physical exam & must be valid through the entire sports season that the athlete will be participating in (a physical may not expire midway through the season).
