Eligibility for Pueo Athletics

No athlete will be allowed to participate in any Maui Prep athletics, including practices or official open gyms, until all four required documents are completed and submitted to HealthyRoster:

Hawaii State Physical Examination For Athletes Form (attached below, completed by hand, upload to HealthyRoster)
Maui Prep Athletic Training Consent Form (electronically completed on HealthyRoster)
Emergency Authorization Signature Form (attached below, handwritten signature required, upload to HealthyRoster)
Maui Prep Concussion Management Acknowledgement (electronically completed on HealthyRoster)

The physical must be completed on the Hawaii Department of Education Athletic Physical Form (available for download on your HealthyRoster account and attached below) by a licensed healthcare provider (medical doctor, osteopathic physician, advanced practice registered nurse, and/or physician assistant).

Important Notes:

Athletic Physicals are valid for 1 year from the date of the exam and must be valid through the entire sports season your athlete participates in.
Your physical must be completed and uploaded to your HealthyRoster account prior to the first day of school.
The Athletic Physical Form is NOT the same as the School Health Record used for admissions. Do NOT upload athletic documents to Blackbaud; the Athletic Department uses HealthyRoster.

Maui Preparatory uses an online data entry system that will help collect current address, emergency contact, insurance, medical alert and health history information, as well as the required forms.

If you're a returning athlete or parent/guardian, log in to your existing HealthyRoster account using your previous login details. Remember to review your medical history, update electronic signatures on required forms, and upload your updated physical.
