Get a jump on required Health Information Forms for your student

The Maui Prep Office of Admissions requires that every student have completed Health Information Forms on file. This includes:

HIDOE Student Health Record
TB Risk Assessment
Vaccination Record
- This must be completed whether you choose to vaccinate or not.

All documents should be uploaded to the Nest by July 15
